For people not used to typewriters, the seated "girl" is moving the carriage - the roll that holds the paper
- either to move the paper to the next line below or to another place on
the same line. Great phone, huh?
The calendar emphasizes the meaning of
"missed a day this month," which seems to refer to menstruation.
Doesn't the standing lady look like Little Orphan
Annie? We'd know for sure if she opened her
eyes.

Note the exact address of the writer, enabling
a reader to write her - that was the point. The Pinkham company had had
problems with the authenticity of people singing
its praises (more here); this helped dispel that
doubt.
